I ran into this issue today and wanted to pass along the solution. In my continuing work as part of the team re-designing my company's public website, I needed to create three subsites within the existing site hierarchy. They are based on the "Publishing Site With Workflow" template. Once created, all the pages in these three subsites were missing the "Publish" button in the page editing toolbar, for myself and the other content owners. We were all able to see the Publish button on the sites that had existed prior to today. The steps needed to publish and approve a page without it were quite cumbersome. I verified that the security settings were the same for the existing sites and the new site, and also that the content types in use were the same. I finally discovered the solution via this MSDN forum thread. Unfortunately, I don't know why the new sites experienced this issue, but Method 1 of the solution worked to restore the "Publish" button to the page.
